Flags and client monitoring
Flags are created automatically when Talked detects a potential risk or concern from intake responses or assessment results, helping you quickly identify clients who may need additional support or prioritisation.
Flags support your clinical judgement, they are not a diagnosis and do not replace your normal clinical assessment. Flags are private to you and not shown to the client.
What triggers a flag
A flag highlights something that may require your attention, for example:
- Suicidal ideation based on intake responses
- Deterioration where scores have significantly worsened compared to a recent previous assessment
- Severe symptoms where scores are in a severe range for an assessment
Flag severities
There are 4 severity levels a flag might have.
- Critical: Immediate safety concern or serious risk. Review as soon as possible and take prompt action if needed.
- High: Significant concern that may require timely follow-up outside the next routine session.
- Medium: Notable risk indicator that should be addressed, typically within the next session or two.
- Info: Low-level signal for awareness. No action may be needed, but acknowledge and monitor.
What you need to do when you see a flag
Review each flag briefly and record an action or acknowledgement. In many cases, this can be a short note such as “will discuss in next session” or “contacted client today”. If the flag is not relevant, you can dismiss it. Once you’ve reviewed or dismissed a flag, it will be removed from the client’s profile.

Click Mark as reviewed once you’ve checked the flag and recorded any relevant notes. Alternatively, click Dismiss to remove the flag if it isn’t relevant.

Once you’ve reviewed the flag, it won’t reappear unless the same risk indicator is triggered again.
